{"id":3678,"date":"2019-02-16T11:43:08","date_gmt":"2019-02-16T11:43:08","guid":{"rendered":"https:\/\/www.mybiosource.com\/learn\/?page_id=3678"},"modified":"2023-03-19T05:50:19","modified_gmt":"2023-03-19T05:50:19","slug":"ventricular-tachycardia","status":"publish","type":"page","link":"https:\/\/www.mybiosource.com\/learn\/ventricular-tachycardia\/","title":{"rendered":"Ventricular Tachycardia"},"content":{"rendered":"<p><strong>Ventricular Tachycardia<\/strong><\/p>\n<p>Ventricular tachycardia (VT) is a heart rhythm disorder that develops because of abnormal electrical signals of the <span id=\"urn:enhancement-6115f0b9-5ecf-4119-bd11-db31703c4573\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ventricles\">ventricles<\/span>, which is the lower chamber of the heart. In a normal scenario, the heart beats around 60 to 100 times a minute which are defined by the signals originating in the atria; the upper chamber of the heart. However, among individuals with this condition, the abnormality causes the heart to beat much faster than normal and out of synch with the upper chambers. Therefore, the heart is not able to pump sufficient amount of blood to the body and the lungs as the upper and lower chambers of the heart are not in synch with each other. If the condition only lasts for a few seconds it may not cause any symptoms, but if it persists, the symptoms may become evident. In the absence of structural <span id=\"urn:enhancement-b27687df-a1a3-4fa0-8726-e06ebb8107b1\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/heart-disease\">heart disease<\/span>, this condition is termed as idiopathic ventricular tachycardia. And, it has accordingly been classified based on the <span id=\"urn:enhancement-ff17e891-c11a-43cc-832f-c836c21a16ee\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ventricle\">ventricle<\/span> of origin, response to <span id=\"urn:local-text-annotation-7hw99qoigktmbmrfytsunajylwu99oi8\" class=\"textannotation disambiguated wl-article\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/pharmacologic-agents\">pharmacologic agents<\/span> and its morphologic features. In most cases, individuals with ventricular tachycardia often have other health conditions such as coronary artery disease, heart valve disease, <span id=\"urn:enhancement-bfd6190d-9a02-4d8a-969c-1831b90fb77a\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/high-blood-pressure\">high blood pressure<\/span> or an enlarged heart. In some cases, the non-heart related reasons for ventricular tachycardia may include an imbalance in <span id=\"urn:enhancement-b5e84fca-91c8-4b1f-a32f-9250d922adb4\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/electrolytes\">electrolytes<\/span>, certain medications, excess alcohol and genetically transmitted conditions.<\/p>\n<p>In rare cases, individuals who already have other heart conditions such as a previous heart attack, the ventricular tachycardia could cause a life-threatening problem such as the sudden <span id=\"urn:enhancement-0cee4b7b-5e8c-40ef-980e-280e8f96af4f\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cardiac-arrest\">cardiac arrest<\/span>. The management and the prognosis of VT can differ greatly based on the individual\u2019s clinical status, the classification of VT and if they already have another <span id=\"urn:enhancement-344f3880-03e5-42ae-a5d0-9784c1364a30\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/heart-disease\">heart disease<\/span>. Individuals with idiopathic ventricular <span id=\"urn:enhancement-716003ad-0ae2-4c18-bed7-c56fb090c1ad\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/arrhythmia\">arrhythmia<\/span> appear to have a better prognosis than those with structural <span id=\"urn:enhancement-7c410997-8f20-4f3f-84cd-bf97f887a0bc\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/heart-disease\">heart disease<\/span> and ventricular tachycardia.<\/p>\n<p><strong>Classification of Ventricular Tachycardia<\/strong><\/p>\n<p>Generally, patients with VT often experience structural <span id=\"urn:enhancement-18352779-aa7d-4a44-a9bc-96c24b3802a5\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/heart-disease\">heart disease<\/span> but those who don\u2019t have any structural heart abnormalities fall into these various categories;<\/p>\n<ul>\n<li><strong>Right Ventricular outflow tract tachycardia (RVOT)<\/strong><\/li>\n<\/ul>\n<p>This is one of the common forms of the outflow tract tachycardia which is normally <span id=\"urn:enhancement-013506ec-a8cb-4758-b433-bf4cd2438689\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/benign\">benign<\/span> and not considered to be life-threatening. However, the affected individual may experience recurrent <span id=\"urn:enhancement-c60bf282-4606-4c38-a2c5-a98350a68b2e\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/palpitation\">palpitations<\/span>, dizziness and very uncommonly loss of consciousness. It seems to be triggered by anxiety or excitement and also with the use of caffeine. Among <span id=\"urn:enhancement-0c658a6b-63a4-4dd9-b1e1-1713683b41f7\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/menopause\">pre-menopausal<\/span> women, hormone appears to play a significant part in triggering this condition. The treatment begins with a reassurance that it\u2019s a <span id=\"urn:enhancement-bf687fe7-fa8f-412e-95ee-a34389693e29\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/benign\">benign<\/span> condition and to avoid stimulants such as caffeine. The beta-blockers appear to be more effective than the calcium channel blocker therapy.<\/p>\n<ul>\n<li><strong>Left ventricular outflow tract tachycardia (LVOT)<\/strong><\/li>\n<\/ul>\n<p>This is an uncommon idiopathic left ventricular tachycardia. This form of tachycardia has a different ECG appearance from the RVOT but some left outflow tract tachycardias present more subtle differences. This condition could be resolved with the use of calcium blockers or beta-blockers. Also, with the advanced technologies like the intracardiac <span id=\"urn:enhancement-ec0f47ae-fefc-44a0-9565-eefcdb1e323a\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/echocardiography\">echocardiography<\/span>, three-dimensional non-contact mapping and magnetic electroanatomic mapping could all be used for the mapping and ablation of LVOT-VT. However, ablation of LVOT-VT may not always be successful and free of complications.<\/p>\n<ul>\n<li><strong>Ventricular tachycardia and cardiomyopathy<\/strong><\/li>\n<\/ul>\n<p>VT in the setting of <span id=\"urn:enhancement-03fd3e95-0e5a-4866-aee4-a9060c415baf\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cardiomyopathy\">cardiomyopathy<\/span> appears to be the common forms of ventricular tachycardia. The <span id=\"urn:local-text-annotation-o98ojtid21izhiuftub9xnuv0vq3fpd2\" class=\"textannotation disambiguated wl-article\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/myocardial-infarction\">myocardial infarction<\/span> could present a central area of scar enveloped by small sections of scar interspersed with living tissue and these channels of living tissue appear to be the pathway for electrical signals to travel. And most VT in the setting of cardiomyopathy appears to take a circular path again and again. So, for individuals with cardiomyopathy and ventricular tachycardia, they could be treated with an implantable defibrillator because of the increased risk of <span id=\"urn:enhancement-e6204c84-5123-473f-bba4-7eab79eabfb3\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/sudden-cardiac-death\">sudden cardiac death<\/span> and those with multiple VT shocks could have <span id=\"urn:local-text-annotation-s9u540bbiqr7zrnsprx5ly3l6ft0ibys\" class=\"textannotation disambiguated wl-article\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ventricular-tachycardia-ablation-the-right-approach-for-the-right-patient\">ventricular tachycardia ablation<\/span>.<\/p>\n<p>&nbsp;<\/p>\n<ul>\n<li><strong>Ventricular tachycardia and <span id=\"urn:enhancement-18ec906d-457a-4668-bd4a-803b17b6103c\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/sudden-cardiac-death\">sudden cardiac death<\/span> in hypertrophic cardiomyopathy<\/strong><\/li>\n<\/ul>\n<p>One of the major causes of death among individuals with hypertrophic cardiomyopathy is the life-threatening arrhythmias although not all cases are susceptible to it. However, in order to determine the increased risk of these serious ventricular arrhythmias, a complete family history, a number of tests such as 24 hours <span id=\"urn:enhancement-df92c35d-dd07-4fa6-9d4c-2c90dabfa42a\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ecg\">ECG<\/span> monitoring, history of prior cardiac arrest will be evaluated by the physician and based on the criteria, the option of an implantable cardioverter defibrillator may be discussed.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Investigations used for Ventricular Tachycardia<\/strong><\/p>\n<ul>\n<li><strong><em><span id=\"urn:enhancement-375e1ac8-6d78-4007-90c6-17b394f0f8d7\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/echocardiography\">Echocardiography<\/span><\/em><\/strong> is the important investigation used for VT. Among individuals with VT related to coronary artery disease, the site of previous infarction and the extent of LV dysfunction are important. Dilated cardiomyopathy and left <span id=\"urn:enhancement-bd3bb13c-ec0f-48c2-b389-eb222dcd777c\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ventricle\">ventricle<\/span> dysfunction among those with longstanding hypertension and diabetes are also considered important causes of VT.<\/li>\n<li><strong><em><span id=\"urn:enhancement-ace38fd5-31a1-490c-b211-6d57f6fb0714\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mri\">MRI<\/span><\/em><\/strong> <strong><em>scan<\/em><\/strong> including the contrast-enhanced imaging is used to study the structural cardiac abnormalities.<\/li>\n<li><strong>Electrophysiological<\/strong> <strong>test<\/strong> is used to confirm the diagnosis and to locate the site of the problem. During this test, catheters are inserted either in the groin or the neck that guides them through the blood vessels to different spots of the heart.<\/li>\n<li><\/li>\n<\/ul>\n<p><strong>Long-Term Management of Individuals with Ventricular Tachycardia<\/strong><\/p>\n<ul>\n<li>VT can be suppressed with beta-blockers and calcium channel blockers<\/li>\n<li>d-Sotalol can be used for individuals with VT who either have structural <span id=\"urn:enhancement-24ed5698-5834-410e-bec2-f0f70b457737\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/heart-disease\">heart disease<\/span> or without it as long as the LV function is not compromised.<\/li>\n<li>Amiodarone exerts its anti-arrhythmic action; however, the long term therapy is associated with high discontinuation rate due to <span id=\"urn:enhancement-715bf7a8-da9f-4349-9004-c6ed2657f623\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/pulmonary\">pulmonary<\/span>, hepatic and <span id=\"urn:enhancement-0dc1904a-4d86-4b31-bdaf-20d02a566789\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/thyroid\">thyroid<\/span>-related side-effects.<\/li>\n<li>Implantable cardioverter defibrillator may be recommended among individuals at risk of having a life-threatening VT episode. This device detects and delivers calibrated electrical shocks when required to restore the normal heart rhythm.<\/li>\n<li>Catheter ablation is used when an abnormal electrical pathway is responsible for increasing the heart rate. This procedure uses the catheters which are guided through the blood vessels and to the heart.
