{"id":4033,"date":"2019-03-11T07:00:56","date_gmt":"2019-03-11T07:00:56","guid":{"rendered":"https:\/\/www.mybiosource.com\/learn\/?page_id=4033"},"modified":"2023-03-19T06:09:54","modified_gmt":"2023-03-19T06:09:54","slug":"necrobiosis-lipoidica-diabeticorum","status":"publish","type":"page","link":"https:\/\/www.mybiosource.com\/learn\/necrobiosis-lipoidica-diabeticorum\/","title":{"rendered":"Necrobiosis lipoidica diabeticorum"},"content":{"rendered":"<p>Necrobiosis lipoidica diabeticorum is a rare, chronic, idiopathic disorder of <span id=\"urn:enhancement-99739605-6d31-4821-9ff3-e6def1ed80fd\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/collagen\">collagen<\/span> degeneration with a <span id=\"urn:enhancement-0a560f0b-c2dc-4b26-ba80-83f28ef34aa3\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/granulomas\">granulomatous<\/span> response, thickening of blood vessel walls, and fat deposition. In 1929, it was first described by Oppehhein, as dermatitis atrophicans lipoidica diabetica. In 1932, Urbach renamed the disease as necrobiosis lipoidica diabeticorum (NLD). It is associated with the risk of ulceration and classically associated with diabetes mellitus, usually, type 1 and affects 0.3% of diabetic patients. The increased prevalence in diabetic individuals is currently questioned. Necrobiosis lipoidica precedes diabetes in up to 14% and appears simultaneously in up to 24% and occurs after diabetes is diagnosed in 62% of cases. Also, no connection has been proved between the level of <span id=\"urn:enhancement-f9e4aa5d-e678-432b-9ab3-c8a025672f4d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/glycemic\">glycemic<\/span> control and the likelihood of developing necrobiosis lipoidica.<\/p>\n<p>The common sites for NLD are legs, abdomen, upper extremities, and scalp. The etiology and pathogenesis of NLD are still controversial, it is thought that <span id=\"urn:enhancement-16573e4d-45fb-4744-b9a2-2d924a105aaa\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/microangiopathy\">microangiopathy<\/span> has an important role. It is more often seen in women with a predominance of 77%. Although it may present in healthy individuals with no underlying disease, other commonly associated conditions are <span id=\"urn:enhancement-dabf4c26-2b84-4d5d-a7ef-5b3354563926\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/thyroid\">thyroid<\/span> disorders and inflammatory diseases, such as <span id=\"urn:local-text-annotation-m0a1irf7uvpg5317wpb028rzmz4jan5i\" class=\"textannotation disambiguated wl-article\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/crohn-disease\">Crohn disease<\/span>, <span id=\"urn:enhancement-d8397d3f-0b86-44c6-955b-a04d716f895e\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ulcerative-colitis\">ulcerative colitis<\/span>, <span id=\"urn:enhancement-d2d18dab-7189-4c1a-bc53-2c1d64306a7d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/rheumatoid-arthritis\">rheumatoid arthritis<\/span>, and <span id=\"urn:enhancement-dd2f494d-be2b-4d60-85b6-38cae9e4d6a7\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/sarcoidosis\">sarcoidosis<\/span>. Few studies are suggestive of both hyperlipidemia and venous reflux, in addition to other pathogenic factors, which trigger tissue damage in the lower legs whereas others conclude that the increased blood flow seen in NLD lesions are suggestive of an ongoing inflammatory process. <span id=\"urn:enhancement-e832b3fa-78bb-4ff8-99f9-eb859d2d6e73\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/histopathological-examination\">Histopathologically<\/span> NLD is characterized by interstitial and palisaded granulomas that involve the <span id=\"urn:enhancement-340ea2c7-e1e3-4d57-8300-c7fb622dbf93\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/subcutaneous\">subcutaneous<\/span> tissue and dermis. The granulomas which are composed of histiocyte are arranged in a tier fashion and are admixed with areas of <span id=\"urn:enhancement-2cbc22a5-389b-4adb-8b64-beb87bfd2239\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/collagen\">collagen<\/span> degeneration. Some of them are multinucleated <span id=\"urn:enhancement-59a87a6b-7033-4efc-9f9b-d84f1780a4a4\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/lymphocytes\">lymphocytes<\/span>, occasional <span id=\"urn:enhancement-018c2b7c-f826-43b1-a083-13a4b00af5a9\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/plasma\">plasma<\/span> cells, and <span id=\"urn:enhancement-a3304afa-d660-46b4-aab3-85d7d5a1c006\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/eosinophils\">eosinophils<\/span>.<\/p>\n<p>Reduction in the number of the intradermal nerve is an additional feature of NLD. The main findings on histopathology are thickening of the blood vessel walls and <span id=\"urn:enhancement-a6e5c929-b259-4b2a-8a27-1ad6d8de73a7\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/endothelial\">endothelial<\/span> cell swelling found in the middle to deep dermis, the characteristics shared with diabetic <span id=\"urn:enhancement-afb92bd6-b634-4bd2-a7f8-20f720680758\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/microangiopathy\">microangiopathy<\/span>. <span id=\"urn:enhancement-5dad31c4-72cf-434e-86f7-be42d6a405e7\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/biopsy\">Biopsy<\/span> in few patients has revealed sandwich-like horizontal layers of necrobiotic <span id=\"urn:enhancement-1d59f9aa-d6f5-46fc-9edb-f175ddbb96eb\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/collagen\">collagen<\/span> alternating with <span id=\"urn:enhancement-f3802a61-3692-4d55-b7d0-cf11b023fa99\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/immune-cells\">inflammatory cell<\/span> infiltrate of <span id=\"urn:enhancement-0af94876-b219-4a6f-a79c-c6ebcd08ed89\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/lymphocytes\">lymphocytes<\/span>, histiocytes, multinucleated giant cells, and plasma cells. <span id=\"urn:local-text-annotation-59r5ngp47x2ujvp4l9ncblv3evu3lviu\" class=\"textannotation disambiguated wl-article\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/direct-immunofluorescence-microscopy-in-the-diagnosis-of-autoimmune-bullous-dermatoses\">Direct immunofluorescence microscopy<\/span> of NLD has demonstrated vascular thickening due to <span id=\"urn:enhancement-366e583a-4eb1-4c8a-88d7-02aefddb5e5e\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/igm\">IgM<\/span>, <span id=\"urn:enhancement-f627af1e-573e-4ffe-96a1-38d3156420d8\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/iga\">IgA<\/span>, C3, and <span id=\"urn:enhancement-fa65a12f-dedb-4993-b83e-481b39e8d7a0\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/fibrinogen\">fibrinogen<\/span> in the blood vessels. In nondiabetic subjects, the vascular changes are not that prominent.<\/p>\n<p>There is no rational therapy for the disease. There are plenty of therapeutic options available for NLD but its treatment is still challenging and sometimes ineffective. First-line treatment includes <span id=\"urn:enhancement-d81fae5f-97e9-4fdd-b48c-8bcda7f6e171\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/corticosteroid\">corticosteroids<\/span>, either topically or intralesionally, and sometimes systemically. Other studies recommend smoking cessation and blood glucose control. Some more therapeutic options are the use of antiplatelet agents, cyclosporine, thalidomide, clofazimine, anti-TNF agents, <span id=\"urn:enhancement-456afccb-2a12-458f-8a5b-b1253ee73d76\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/fumaric-acid\">fumaric acid<\/span> esters, <span id=\"urn:enhancement-3c11d1ae-b55e-4f05-a3c9-fb205dd15026\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/photodynamic-therapy\">photodynamic therapy<\/span>, hydroxychloroquine, and <span id=\"urn:enhancement-41936887-77c4-4f34-9923-a506c9f6af62\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/tacrolimus\">tacrolimus<\/span>. Tacrolimus induces suppression of the <span id=\"urn:enhancement-b36084ca-b804-4a17-ace9-81d633eb6aae\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/granulomas\">granulomatous<\/span> infiltrate including the reduction of chemotactic activity of fibroblasts and the inhibition of <span id=\"urn:enhancement-01cde2a8-de7b-4e59-aff2-241152573103\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/collagen\">collagen<\/span> synthesis, <span id=\"urn:enhancement-7c47f7b9-f336-4aad-afc8-d19a179cce5d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/anti-inflammatory\">anti-inflammatory<\/span>, and immunomodulatory effects. Subsequently, tacrolimus is thought to be effective at the early inflammatory stages of NL. Significant insights have been made into its pathophysiology and treatment of NLD even though its details are still elusive.
