These pests are common even in the cleanest of crowded urban areas and older dwellings. The proteins found in cockroach saliva are particularly <span id=\"urn:enhancement-1f73ba03-7a7c-4b17-a2b0-22bdd770f915\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergenic<\/span> but the body and droppings of cockroaches also contain <span id=\"urn:enhancement-94d3d82e-47f8-4ac3-af2b-8079459c64f5\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergenic<\/span> proteins. By now it is well proven that cockroach carries some potent <span id=\"urn:enhancement-e4ef376c-af0c-4c52-8af3-61fbc46afa06\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergens<\/span> which are sufficient to cause respiratory diseases.<\/p>\n<p>In 1943 cockroach <span id=\"urn:enhancement-feda7d66-a5ce-495a-b6c1-9d9287c0b20e\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergic\">allergy<\/span> was first reported, when skin rashes appeared immediately after the insects crawled over patients skin. It was 1959 when skin test confirmed cockroach <span id=\"urn:enhancement-3c4c7f1a-53ab-46bb-a6a8-68761a8448e3\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergic\">allergy<\/span> and in 1970 studies made it clear that patients with cockroach allergies develop acute <span id=\"urn:enhancement-f70414f0-a049-49b1-ab8a-d2db8baf5bd4\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/asthma\">asthma<\/span> attacks. Continuous exposure to cockroach <span id=\"urn:enhancement-a99f1f59-7c48-43b2-9e01-b2fb2de091e7\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergens<\/span> in threshold amounts establishes a persistent inflammatory response with in the airways.<\/p>\n<p>Two cockroach species namely <em><span id=\"urn:enhancement-b5292ed3-416c-4017-88fd-58d12c108da8\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/periplaneta-americana\">Periplaneta americana<\/span><\/em> and <em>Blattella germanica<\/em> are important <span id=\"urn:enhancement-d8463a33-d288-4881-a34c-6cd6948bd735\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergen<\/span> sources world-over. They play a major role in etiology of <span id=\"urn:enhancement-acbede08-fc65-409c-9133-c08724484f44\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergic\">allergic<\/span> <span id=\"urn:enhancement-d07819f9-739b-43d0-9384-65b6a4dba27b\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/asthma\">asthma<\/span> and rhinitis. Cockroach extracts are rich in <span id=\"urn:enhancement-856a876d-6408-4a2a-a928-ad333232e4c2\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/proteases\">proteases<\/span> and induce direct proinflammatory effects on the airway epithelial cells, attributable to <span id=\"urn:enhancement-1fe7df16-1fa1-48d9-898f-af0e43467523\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/serine\">serine<\/span> <span id=\"urn:enhancement-3290fd73-245b-4e04-a2ff-722f324c71e2\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/proteases\">protease<\/span> activity. <span id=\"urn:enhancement-7dc003bf-403f-427d-b11f-b2e0c0bf38f4\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergic\">Allergic<\/span> diseases have become a serious public health concern, leading to great socioeconomic burden. <span id=\"urn:enhancement-309ab3fb-0d7f-4632-8277-fc1a2b6496b2\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergic\">Allergic<\/span> symptoms can be mitigated by pharmacological treatment but symptoms reoccur rapidly on withdrawal of drugs.<\/p>\n<h3><strong>Cockroach allergen immunotherapy<\/strong><\/h3>\n<p><span id=\"urn:enhancement-f1ce56cf-568e-4ed5-a20d-9b5450355fa6\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergen-specific-immunotherapy\">Allergen-specific immunotherapy<\/span> (SIT) prevents progression of disease and has long-lasting effects. SIT which involves, the gradual administration of increasing amounts of\u00a0<span id=\"urn:enhancement-0cd5f533-f1ef-43b5-a910-865b1095a36d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergen<\/span>\u00a0for the purpose of inducing protective immunological changes\u00a0selectively modulates the\u00a0<span id=\"urn:enhancement-2b152b5d-cca5-4e49-a093-148b3dfd4afa\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergen<\/span>-specific\u00a0immune response. SIT has been shown to modulate <span id=\"urn:enhancement-48d3359a-7b7f-4910-8523-1ffdd559e2fd\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergen<\/span>-specific antibody responses with a decrease in the <span id=\"urn:enhancement-49aa0e14-90dc-4bd3-9e00-be58619c9fce\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ige\">IgE<\/span>\/<span id=\"urn:enhancement-4cdee775-7f86-42cb-bcc5-5d4aef4d3f0a\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/igg4\">IgG4<\/span> ratio leading to reduction in the recruitment and activation of pro-inflammatory cells in target mucosa. SIT results clinical improvement which has been found to correlate with increase in <span id=\"urn:enhancement-c4fa9283-5c15-4df2-b31d-63cb7346bed4\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/igg4\">IgG4<\/span>. During the first few months of immunotherapy there is increase in <span id=\"urn:enhancement-a24a76ee-21b9-4a75-a67d-772eb7e32bfc\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergen<\/span>-specific <span id=\"urn:enhancement-27ae4a76-96c5-41b5-8b35-ef881f86cc39\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ige\">IgE<\/span> which further slows down on long-term treatment. The immunotherapy-induced early increase in specific <span id=\"urn:enhancement-d2f14807-ec74-4235-8483-3066d7fa6440\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ige\">IgE<\/span> is nonpathogenic and may be a result of a reduced <span id=\"urn:enhancement-b0434a7a-4f1f-4e01-99a5-bb0ff04c463a\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/affinity\">affinity<\/span> to the <span id=\"urn:enhancement-3f316d7e-3550-4cbe-a221-b06c43dcdc3c\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/fc\">Fc<\/span> receptor or to a switch from <span id=\"urn:enhancement-020cf354-dda6-4f6f-ae39-b20face265b5\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ige\">IgE<\/span>+ to <span id=\"urn:enhancement-994bd510-d47f-4169-8c8f-ea62b1cc34ce\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ige\">IgE<\/span>\u2212.<\/p>\n<p>Many reports have suggested reduction in <span id=\"urn:enhancement-624800d6-335b-4132-9f22-2b33694a30a9\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergen<\/span>-specific <span id=\"urn:enhancement-3e88e0be-4c9f-4910-b58b-e5a9d8fdc425\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ige\">IgE<\/span> levels and induction of <span id=\"urn:enhancement-e752a5b5-9516-4cc9-a737-23e147acfa44\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergen<\/span>-specific <span id=\"urn:enhancement-99753999-4fef-4ea7-bf8e-c4d992825360\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/igg4\">IgG<\/span> subclasses by SIT. Cockroach IT has also reported improvement in clinical and immunological status of <span id=\"urn:enhancement-624d2a10-3036-45f2-a21b-75087028d877\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/asthma\">asthma<\/span> and rhinitis patients. However, commercial cockroach extracts used for diagnosis and treatment remain partially standardized.\u00a0 Injection IT with whole body extract induces clinical response in large number of patients, however IT with such extract may result in additional sensitivity to irrelevant components and can hamper determining the optimal maintenance dose.<\/p>\n<p>Over the years, many cockroach <span id=\"urn:enhancement-011e2452-fb8b-4ddb-850c-85c87127e6f1\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergens<\/span> have been identified, sequenced, and produced as biologically active recombinant proteins. A \u223c28-kDa major <span id=\"urn:enhancement-edf2938a-7cc7-4011-9adf-d44ab072edcc\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergen<\/span> of <em>P. americana<\/em> with proteolytic activity designated as Per a 10 was reported. In mice model proteolytic activity of Per a10 was observed to enhance <span id=\"urn:enhancement-9a2e4470-75d1-4d98-87e5-f2740df569e0\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ige\">IgE<\/span> effect. The inhibition of the proteolytic activity was found to significantly reduced the Per a 10 associated <span id=\"urn:enhancement-ec83b7a0-1358-4d84-a3e3-f0065bd47933\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/eosinophilic\">eosinophilic<\/span> airway inflammation and related manifestations of <span id=\"urn:enhancement-8a390de7-567d-42f5-a2e2-8de4cfb984b6\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergic\">allergic<\/span> airway disease.<\/p>\n<h3><strong>\u00a0<\/strong><strong>Component resolved cockroach immunotherapy<\/strong><\/h3>\n<p>For accurate diagnosis of sensitization correct prescription of specific <span id=\"urn:enhancement-8da4571d-b94d-458d-850a-b7a47262ee5d\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/immunotherapy\">immunotherapy<\/span> is prerequisite. Diagnosis with ill-defined natural <span id=\"urn:enhancement-db760f05-5606-4c75-894a-3a46737c2193\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergen<\/span> extracts can be erroneous and inadequate. Purified <span id=\"urn:enhancement-d87c68bc-9489-419b-881e-35fbd710f7ad\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergens<\/span> are important in terms of component-resolved diagnosis and SIT of <span id=\"urn:enhancement-84778a4a-e7c6-4569-b799-0646ebab1e1b\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergic\">allergic<\/span> diseases. In the process of assessing effects of component resolved cockroach <span id=\"urn:enhancement-51b028dc-b159-4306-bd22-f1b16c1dc5c6\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/immunotherapy\">immunotherapy<\/span> Srivastava et al investigated the immune modulation induced by purified major <span id=\"urn:enhancement-471113b1-8da7-479c-9e8e-572be1151f86\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergen<\/span> (Per a 10, in its active and inactive form) of <em>P. americana<\/em> in mouse model of <span id=\"urn:enhancement-a52c30f0-9743-4caf-ae53-81e2a329fd2a\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergic\">allergic<\/span> airway disease.<\/p>\n<p>Cockroach <span id=\"urn:enhancement-1bb095b3-4b5d-484b-aed9-542ea82c2b43\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergen<\/span> sensitized mice were treated with purified cockroach <span id=\"urn:enhancement-ace1ef80-bb43-4b56-bcb5-e66e14016c4b\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergenic\">allergen<\/span> Per a10 in its active and inactive form. <span id=\"urn:enhancement-72909324-2008-400f-bfbf-9490cff84e69\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/immunoglobulins\">Immunoglobulins<\/span> were measured in <span id=\"urn:enhancement-cddeae32-fa5f-47fb-9b24-de1c5dfe6394\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/serum\">serum<\/span> and <span id=\"urn:enhancement-907f33f7-72d0-4610-beda-dd0771bcbdcf\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cytokines\">cytokines<\/span> in bronchoalveolar lavage fluid and splenocyte culture supernatant. \u00a0<span id=\"urn:enhancement-09663089-e6b7-4979-abe2-2b8c35b24608\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/immunotherapy\">Immunotherapy<\/span> with Per a 10 (active and inactive) in Per a 10-sensitized mice showed significant reduction in <span id=\"urn:enhancement-4c133b95-a89a-4514-8db7-74533086c3cb\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/serum\">serum<\/span> specific <span id=\"urn:enhancement-c5ee932b-882e-4833-a2c1-8aff61e02e4c\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ige\">IgE<\/span> levels which was more evident in inactive Per a 10 treated mice. Significant reduction was observed in <span id=\"urn:enhancement-f18f825e-25f8-4e0a-9048-55010ad41a55\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/il\">IL<\/span>-4 levels whereas <span id=\"urn:enhancement-d03eeb6f-1f44-4df3-86aa-35a2fade3dc2\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/il\">IL<\/span>-10 levels showed significant increase in BAL fluid and culture supernatant post-IT with Per a 10 (<span id=\"urn:enhancement-62435264-32ca-4882-a496-bc1027f7a969\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/proteolytically\">proteolytically<\/span> active and inactive).<\/p>\n<p>Most importantly, IT with inactive Per a 10 showed significant reduction in <span id=\"urn:enhancement-648d3284-66f6-4356-91a5-16f4eb2b61da\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/il\">IL<\/span>-4 level accompanied by maximum increase in <span id=\"urn:enhancement-3f3b3e85-2e79-43be-9694-826eca43d4f4\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/il\">IL<\/span>-10 level.\u00a0 Lung histology also demonstrated significant reduction in peribronchial and perivascular infiltration of eosinophils, in mice treated with active and inactive Per a 10.
