There are more than hundred types of human papilloma <span id=\"urn:enhancement-65c494c5-01d2-435a-a86c-423c17671d8e\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/virus\">virus<\/span> and around 40 are estimated to affect the genital area appearing around the vagina, vulva, tip of the penis and the anus. Also known as <span id=\"urn:enhancement-b6d30ef3-8f73-4f5e-a50e-5a2b0427cf08\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/condylomata-acuminata\">condylomata acuminata<\/span> (external genital warts), it is considered one of the common forms of <span id=\"urn:enhancement-7cf4794d-7493-46c4-afa2-8188582d2021\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/sexually-transmitted-diseases\">sexually transmitted diseases<\/span> globally. Warts can vary in its appearance as either flat, cauliflower or resemble raspberries. It can begin as pink or red growths continuing to reach four inches across thus affecting normal sexual intercourse. Majority of the genital warts are normally asymptomatic but based on their location, it can be painful. In most of the cases, it can resolve on its own within a few years but the immunocompromised individuals such as <span id=\"urn:enhancement-8d34662a-acab-4a07-a389-e008e09e1ae4\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hiv\">HIV<\/span>-positive people are the highest burden of <span id=\"urn:enhancement-ea5b71b4-ec8d-4370-a0ad-3e202ccff598\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> and are particularly susceptible to recurrences and treatment failure. <strong>Virology<\/strong> <span id=\"urn:enhancement-75ebdaca-d1a8-4510-a74d-bd3292ca9861\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> is a <span id=\"urn:enhancement-a6eace60-fc2b-45f1-8235-37cc3a09e3cd\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/single-stranded\">double-stranded<\/span> deoxyribonucleic acid <span id=\"urn:enhancement-0e124a22-6006-4935-afe7-40fd78170e45\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/virus\">virus<\/span> belonging to the family Papovaviridae. In search of the appropriate cellular host, the <span id=\"urn:enhancement-55070f79-460e-4bb5-a85a-c14b6884ec2b\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/virus\">virus<\/span> enters the cutaneous and mucosal epithelium. The mucosa of the genital tract includes the vulva, vagina, <span id=\"urn:enhancement-ebb22307-6f2f-40b5-ad38-b33a960bcb69\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cervix\">cervix<\/span>, <span id=\"urn:enhancement-d67af01a-935d-4d89-9fc6-d59e0b38ac14\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penile\">penile<\/span> shaft, periurethral and <span id=\"urn:enhancement-ecf138df-56a1-43ad-bd00-0c79aeb34244\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/perianal\">perianal<\/span> regions. The low-risk <span id=\"urn:enhancement-b3a3e4c1-1a06-44c1-9e79-1fc63e2097a7\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> strains undergo replication separately from the host cell DNA in contrast to the high-risk <span id=\"urn:enhancement-9497a1a3-04eb-4938-94fb-fff2273bde25\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> strains that will incorporate its DNA into the host\u2019s genomic material. This integration of DNA results in the initiation of the E6 and E7 genes stimulating the transcription of oncoproteins. As a result, it will inactivate tumor suppressor genes causing <span id=\"urn:enhancement-27b4527c-11db-4aef-9a39-5012cba12100\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cell-proliferation\">cell proliferation<\/span> and the risk of malignant progression.<\/p>\n<p><strong>HPV associated Anogenital Cancer<\/strong><\/p>\n<p>Although the anogenital cancers are generally uncommon, there is a growing incidence currently which is attributed to the increasing <span id=\"urn:enhancement-af3ce50c-53a5-4510-b218-0168673cb491\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> transmission. According to CDC report, an average 38,793 <span id=\"urn:enhancement-cdfe36af-615b-4bb9-9173-eac2c823405c\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span>-associated cancers are detected every year (23,000 among females and 15,793 among male population) and the most common of these cancers are the cervical carcinomas at 11,771 females. <span id=\"urn:enhancement-b1b0840d-3089-4606-9a8b-849bf7de4e6a\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> is believed to be responsible for more than 90% of anal and cervical cancers, 70% of vaginal and vulvar cancers and an estimated 60% of <span id=\"urn:enhancement-c0f8f7b8-2150-47b8-9dcc-af400ce8e7ac\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penile\">penile<\/span> cancers. The high-risk <span id=\"urn:enhancement-4581d9d1-9314-4fb4-917a-40447fcfe032\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> viral genotypes are 16, 18, 31, 33, 35, 39, 45, 51, 52, 56, 58, 59 and 68, however, the most commonly isolated carcinogenic <span id=\"urn:enhancement-4db10c64-a5fa-4c3f-9a00-b6812750cd6d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> types are 16 and 18 accounting for 70% of all cervical cancers globally. Most of the risk factors associated with the development of anogenital cancer include receptive anal intercourse, a sexually transmitted disease, genital hygiene, persistent <span id=\"urn:enhancement-17048e38-4380-4310-8b6a-93ee5b0fc345\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> infection, immune deficiency <span id=\"urn:enhancement-8cb44cc2-a767-4e85-90d8-bd1853ac425d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syndrome\">syndrome<\/span> and cigarette smoking.<\/p>\n<p><strong>Diagnosing HPV and screening for HPV associated cancers <\/strong><\/p>\n<p>Currently, there are no blood tests or swab to detect <span id=\"urn:enhancement-66b686b6-f911-4c8d-b63d-cdc6dc09a60c\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> and diagnosis is based on visible warts on the genital skin. There are no routine screening tests for <span id=\"urn:enhancement-417dd329-8309-47e9-a777-c8925ab27673\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> related cancers other than <span id=\"urn:enhancement-5c9777d5-a3c7-4836-b2a2-e186207ad665\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cervical-cancer\">cervical cancer<\/span>. Majority of women presenting abnormal smear have had <span id=\"urn:enhancement-90bb33ee-6014-4915-9b1f-0d99ae5a9062\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> of the vagina, <span id=\"urn:enhancement-b1dd72e5-02e6-4dda-9293-bdb19be4a210\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cervix\">cervix<\/span> or the vulva but cervical screening is not a test for an <span id=\"urn:enhancement-542d4f69-1366-40ed-acda-0c7bd7ca1e4b\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> infection. <span id=\"urn:enhancement-1e38bc12-538a-4775-8a7d-837df3f14df3\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cervical-cancer\">Cervical cancer<\/span> is usually detected by the routine <span id=\"urn:enhancement-ece0462a-ffd3-45b9-a715-1b59e308c56d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cervical-cancer\">cervical cancer<\/span> screening and the <span id=\"urn:enhancement-b586b0c5-756a-4256-a6fe-6f35f4366aee\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> DNA test may also be done to detect the type of <span id=\"urn:enhancement-3d00a7bc-b66f-423a-84da-c328a4011fe1\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> on the <span id=\"urn:enhancement-efdb00bc-7903-478a-8a08-9d0d1d0d94bb\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cervix\">cervix<\/span>.<\/p>\n<p><strong>Regimen for Genital Warts<\/strong><\/p>\n<p>The treatment of genital warts poses a therapeutic challenge and is focused on the removal of the lesions rather than elimination of the viral infection. There are several forms of therapeutic options available such as the use of <span id=\"urn:enhancement-4f1ea859-1ebe-4027-a68c-1a85b75f9a31\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cytotoxic\">cytotoxic<\/span> agents, physical ablation and immunomodulation, but no definitive therapy is considered an ideal treatment of genital wart for an overall effectiveness. Most forms of the treatment are uncertain of complete cure achievement with the possibility of recurrence of lesions. Here are some results of the study initiated for the treatment of genital warts; <em><span id=\"urn:enhancement-e148e972-49bd-4982-9a48-327cc5f0c63f\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/imiquimod\">Imiquimod<\/span> 5% cream<\/em> \u2013 an open-label <span id=\"urn:enhancement-a0d38f2e-4ca6-4f76-8f4d-0f3fcf16593d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/phase-ii\">phase ii<\/span> pilot study concluded that the preliminary results recommend a 4 week treatment course of five percent imiquimod cream which is effective for the treatment of genital warts. In a comparative study, the efficacy of topical <em>podophyllin and <span id=\"urn:enhancement-bdcb32bd-c1b8-41a9-8f0e-f24e09d7e6ce\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/podophyllotoxin\">podophyllotoxin<\/span><\/em> were compared as the treatment option among men with genital warts. The study illustrated that 82.8% of men using podophyllin and 89.1% of <span id=\"urn:enhancement-120bc192-fd22-4042-b192-d72bda2fb8fd\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/podophyllotoxin\">podophyllotoxin<\/span> group were completely cured although at the end of a 6 months follow-up, some evidence of recurrence of warts was witnessed. <em><span id=\"urn:enhancement-7a91fd5e-51b7-4716-b862-b13085c6efdb\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/interferons\">Interferon<\/span><\/em> \u2013 It is widely used for its antiproliferative, immunomodulatory and <span id=\"urn:enhancement-18a72de9-dea6-448c-9ec4-aa687393d5d9\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/antiviral\">antiviral<\/span> properties. It is shown to be effective against <span id=\"urn:enhancement-45e8568a-8dfa-4d90-81b9-be7c6f566597\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> <span id=\"urn:enhancement-efd066fa-89b7-4cf2-aadc-807529769da3\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/in-vivo\">in vivo<\/span>, vitro and its ability to destroy extra chromosomal viral DNA from infected cells. Locally-used interferon can be effective by both improving and reducing the recurrence rate. <em>Trochloroacetic acid (TCA) 80-90% solution (Grade B)<\/em> \u2013 It is a destructive acid that burns and erodes the skin which is prepared in 80-90% solutions. Though successful treatment can be attained in a single dose, several applications are usually needed. <em>Surgical intervention<\/em> \u2013 Large genital warts can be surgically removed by excision and in the case of warts present on the <span id=\"urn:enhancement-de882bfc-93d1-448b-89a1-6b2239c938bb\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cervix\">cervix<\/span>, laser or loop electrosurgical excision is required.<\/p>\n<p><strong>Study of Liquid Nitrogen Cryotherapy as Monotherapy for Condyloma Acuminata (CA)<\/strong><\/p>\n<p>In a need to identify a monotherapy as an effective treatment for CA, a comparative study was conducted. The study consisted of two groups; as monotherapy, one group was subjected to liquid nitrogen cryotherapy while the other group was given liquid nitrogen in combination with podophyllin. The study concluded cryotherapy as a safe and effective treatment of multiple warts and in combination with podophyllin was considered even more effective. <strong>Special Considerations During Pregnancy <\/strong> According to CDC, administration of drugs such as <span id=\"urn:enhancement-5680eccb-5ed5-410e-81f6-3d34939206c3\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/imiquimod\">Imiquimod<\/span>, podophyllin and podofilox during pregnancy should be avoided. In some rare cases, <span id=\"urn:enhancement-1ce8217e-8803-4a66-97b1-2fd0b146755d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> types 6 and 11 can result in <span id=\"urn:enhancement-4ba40369-479c-4dc1-8e5b-dfc95f5ec4b1\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/respiratory-papillomatosis\">respiratory papillomatosis<\/span> among infants but the route of transmission is not entirely known. Also, cesarean should not be conducted with the intention of preventing the transmission of <span id=\"urn:enhancement-78f26f4b-17b5-44e3-92a9-3cc6560338a1\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> infection to the infants and should only be performed in the case of pelvic obstruction. <strong>Prevention measures<\/strong> <span id=\"urn:enhancement-c6c079dc-a453-4df1-96e4-9e12538734c8\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/gardasil\">Gardasil<\/span> as the vaccine against human <span id=\"urn:enhancement-b52e720a-fe4d-476b-b68e-46658834a066\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/papillomavirus\">papillomavirus<\/span> was approved by the United States <span id=\"urn:enhancement-f6cd5c7c-3e4c-4a7b-bb7d-5c3fe4ef18fa\" class=\"textannotation disambiguated wl-organization\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/food-and-drug-administration\">Food and Drug Administration<\/span> (FDA) in 2006 for children as early as nine years of age. It was used to protect against <span id=\"urn:enhancement-ca790f91-44fc-4c89-8a1f-0f8a5fc57c20\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> types 6,11,16 and 18 as the majority of cervical cancers was caused by types 16 and 18 while 6 and 11 were responsible for genital warts in 90% of the cases. In 2014, <span id=\"urn:enhancement-99c26e16-61d4-4a59-9945-f3ed815f0668\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/gardasil\">Gardasil<\/span> 9 was approved by the FDA to protect against the first generation of <span id=\"urn:enhancement-32602f18-3331-4610-aa38-d546aaa599d5\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/gardasil\">Gardasil<\/span> along with other five <span id=\"urn:enhancement-c2c160e7-7cd9-47a0-b881-a89b797aeb65\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hpv\">HPV<\/span> strains, 31, 33, 45, 52 and 58. The Advisory Committee for <span id=\"urn:enhancement-cd11c160-695c-4d19-b368-9ca6ba86a6c2\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/immunization\">Immunization<\/span> Practices (ACIP) recommends routine vaccination for women between the ages of 9 to 26 although high efficacy was noted up to the age of 45.
