Conclusion<\/strong><\/p>\n<hr \/>\n<p>Molloscum contagiosum (<span id=\"urn:enhancement-b295dc81-d2cb-4381-8a5e-4a59a4d4ec04\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span>) is a <span id=\"urn:enhancement-ad32e379-d2ca-4f95-a6a7-d6de437ed2fd\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/benign\">benign<\/span> condition limited to <span id=\"urn:enhancement-9376f46e-d739-442d-b9b2-6957cbb4f7ed\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/keratinocytes\">keratinocytes<\/span> typically presenting pinkish purple color lesions appearing dome-shaped. <span id=\"urn:enhancement-a9e60eee-9745-4369-a1d7-936fd84519d1\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> is evident globally and is particularly common in children with lesions developing anywhere in the body. Among adults, it is observed as a sexually acquired condition affecting the genitals, pubic region, upper thighs and buttocks. The transmission occurs by two routes; either by the direct skin-to-skin contact or indirect contacts such as underclothes or towels. In utero and prepartum transmission of the infection has also been reported causing congenital <span id=\"urn:enhancement-e47d6840-c082-41f3-9164-ea31ff879fb8\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/molluscum-contagiosum\">molluscum contagiosum<\/span>.<\/p>\n<p><strong>Clinical Presentation<\/strong><\/p>\n<p>The infection starts as a small single or multiple, white <span id=\"urn:enhancement-532c8ff4-514d-40fa-97fa-bd8f7b8d751d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/papules\">papules<\/span> developing particularly on the genital region and the face. The lesions often present pearly appearance and are usually smooth and firm. They can range in size from that of a pin-head to 2-5 millimeters in diameter and in some cases may become sore, itchy and swollen. The incubation period is estimated to be somewhere between 2 weeks to 6 months. Atopic <span id=\"urn:enhancement-e5d0e3d0-5e87-4e53-ab2b-864274ccdf45\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/eczema\">dermatitis<\/span> is considered as a risk factor of developing <span id=\"urn:enhancement-673e371b-cba0-4c82-b13b-0fa589e531ce\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> because of the <span id=\"urn:enhancement-0aa072ca-101a-4590-b416-0cabd33d4a35\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/immune-cells\">immune cell<\/span> dysfunction in atopic skin. <span id=\"urn:enhancement-148ccea5-12bd-4deb-99a7-ca8455f72070\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/weakened-immune-system\">Immunocompromised<\/span> patients such as <span id=\"urn:enhancement-84f7890b-3b90-4677-b0cc-900657064ace\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hiv\">HIV<\/span> positive individuals can continue to develop giant lesions more than 15mm in diameters resistant to standard therapy. <span id=\"urn:enhancement-c0b5bc7c-9dfb-45bb-95f8-7800c6ab1ca7\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> is recently classified as common skin lesions affecting children; the sexually transmitted lesions developing on the genitals, abdomen and inner thighs; and the diffuse and recalcitrant eruptions affecting <span id=\"urn:enhancement-6e255c7d-457d-43cd-b0ed-5367facf9f7c\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/immunosuppressive-therapy\">immunosuppressed<\/span> disorders.<\/p>\n<p><strong>Virology and Pathogenic Mechanisms<\/strong><\/p>\n<p><span id=\"urn:enhancement-0877f0c1-7443-47c4-b1ca-6e0bde2e3960\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> is a double-stranded DNA poxvirus and four major subtypes are identified based on <span id=\"urn:enhancement-54a31474-5635-48fd-ab7c-fbb7b9f752b6\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/dna-analysis\">DNA analysis<\/span>; <span id=\"urn:enhancement-b5db89ab-3490-4c26-84ec-f53629cb302b\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mcv\">MCV<\/span>-1 (98% affecting children), <span id=\"urn:enhancement-8a4717eb-e243-448e-954f-cd8603ef4bed\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mcv\">MCV<\/span>-2 (causes skin lesions particularly among <span id=\"urn:enhancement-30ac1d26-fb96-4236-a981-ace045eaf21b\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hiv\">HIV<\/span> individuals), <span id=\"urn:enhancement-b8c66756-448f-4445-b165-b26371e226a3\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mcv\">MCV<\/span>-3 and <span id=\"urn:enhancement-689e9f1e-43ab-4cb6-bd11-da64a2c14226\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mcv\">MCV<\/span>-4 prevalent in Asia and Australia. Molloscum contagiosum virus (<span id=\"urn:enhancement-a503bc6d-6443-430c-8217-5132a7fa3b1f\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mcv\">MCV<\/span>) proliferates within the <span id=\"urn:enhancement-0998d5e3-42df-4522-b92b-6664d7066b42\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/follicular\">follicular<\/span> epithelium separating layers 1 to 3 of <span id=\"urn:enhancement-518bdad5-af96-41a6-9c6f-a02aece1f870\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cd34\">CD34<\/span>+ stromal cells. <span id=\"urn:enhancement-f6e6f3e3-ee0e-4f4c-bcca-e7fb676890aa\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mcv\">MCV<\/span> produces proteins affecting the <span id=\"urn:enhancement-a1f021a8-5847-4bc5-a766-c71e8604502a\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/antiviral\">antiviral<\/span> <span id=\"urn:enhancement-df1c0f9e-2b40-43b1-805f-ed7ad6fa76b7\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/immunity\">immunity<\/span>, hence the development of innate immune response becomes affected. As <span id=\"urn:enhancement-17fe3e08-ed1f-4247-8353-221a7cb7d31b\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mcv\">MCV<\/span> proliferates cytoplasmically, the internal organelles are dislocated as the infected cell continues to grow. The molluscum bodies are the viral particles that develop within the <span id=\"urn:enhancement-1909600f-607e-4481-8bd4-e3ac8cd6c3e6\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cytoplasm\">cytoplasmic<\/span> vacuoles and are detected with special stains like <span id=\"urn:enhancement-80fb13ea-b031-4942-8852-1007ab96c1fa\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/phosphotungstic-acid\">phosphotungstic acid<\/span>&#8211;<span id=\"urn:enhancement-c0757b17-0283-49e5-be9f-3ba47e22c0a3\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hematoxylin\">hematoxylin<\/span> preparation.<\/p>\n<p><strong>Inflammatory Reactions in Children<\/strong><\/p>\n<p>The inflammatory response is common among children causing <span id=\"urn:enhancement-28a3ed7b-a3b3-4afb-947f-3f6be7bbb4bb\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/pruritus\">pruritus<\/span> and pain. There are several forms of inflammatory reactions developing in association with <span id=\"urn:enhancement-a87cdd0f-8696-4834-8140-2c5fa81a47fe\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span>. The inflamed lesions associated with <span id=\"urn:enhancement-b3acac00-606a-4d5c-ac92-aa5161bc3dbf\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> are characterized by swelling including <span id=\"urn:enhancement-2a37fd30-4445-47ca-858d-1eb64650791d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/pustular\">pustular<\/span> lesions. In a study conducted at a <span id=\"urn:enhancement-ee4afc9f-366a-41f5-86ef-d4b6771d1845\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/health-care\">tertiary care<\/span> pediatric dermatology practice, of all the 696 <span id=\"urn:enhancement-a6526fe5-390d-4464-a0b5-516d0aeea260\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> affected patients, molluscum <span id=\"urn:enhancement-ac99f390-98ea-4b8c-8aee-6238be2fa76a\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/eczema\">dermatitis<\/span> was identified in 38.8% of the cases, inflamed <span id=\"urn:enhancement-f23f28d5-0778-487b-84cd-e681adaa0530\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> lesions accounted for 22.3% and Gianotti-Crosti syndrome-like reactions were evident among 4.9%. The Gianotti-Crosti syndrome-like reactions is characterized by an eruption of monomorphous, papulovesicles or <span id=\"urn:enhancement-2af8147b-c720-4e77-819a-3f1118ea2e31\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/erythematous\">erythematous<\/span> <span id=\"urn:enhancement-da88034f-d869-464e-81eb-7933343319d5\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/papules\">papules<\/span> separate from <span id=\"urn:enhancement-2d2d835e-cb93-4607-ac90-48e2c3135007\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> lesions and developed among 34 patients during the <span id=\"urn:enhancement-a9f34952-1a85-44a1-9745-6d98ec08a9ec\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span>. Among 13 patients, the mean duration of GCLR was identified as 6 weeks and the treatment with topical corticosteroid greatly improved the condition within one week. As the inflammatory response among children is poorly studied, an improved understanding of the pathogenesis of the associated inflammatory reaction of <span id=\"urn:enhancement-a0dba46c-4fb8-482d-8318-46a89819bed9\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> can assist with the development of effective treatment.<\/p>\n<p><strong>Evaluation (screening) and differential diagnosis<\/strong><\/p>\n<p>The diagnosis is primarily based on the characteristic feature of the lesions and in the case of a diagnostic challenge, <span id=\"urn:enhancement-358b9e97-b554-430b-ac87-99e53ee7a1a3\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/dermatoscopy\">dermatoscopy<\/span> may be beneficial by detecting a central white to a yellow amorphous area with branched vessels. If an atypical infection is identified, a <span id=\"urn:enhancement-4211642c-d831-4584-b8fb-a7b1c8ebdae2\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/biopsy\">biopsy<\/span> may be performed to detect the <span id=\"urn:enhancement-98336664-9289-4641-b1e5-75e37046342e\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/histopathological-examination\">histopathologic<\/span> features. When diagnosis difficulty persists, <span id=\"urn:enhancement-881f9a66-ce07-4ae7-b1f3-f4f840b6b8ed\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/polymerase\">polymerase<\/span> chain reaction for molluscum can be used although it is not routinely performed.<\/p>\n<p>In some cases, the <span id=\"urn:enhancement-06c76d22-cae8-408d-aef3-cb9c5667bc6c\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> may be incorrectly diagnosed as <span id=\"urn:enhancement-98bf56b6-f48f-4057-bcc9-8200fc235c62\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/cysts\">cysts<\/span>, <span id=\"urn:enhancement-550e201b-ea37-4bda-aed8-cbbe3b5fe4e7\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/basal-cell\">basal cell<\/span> <span id=\"urn:enhancement-e75e7428-ab6e-4154-9a85-47fddb2f99ee\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/carcinoma\">carcinoma<\/span>, cutaneous horn and keratoacanthoma.\u00a0 The genital <span id=\"urn:enhancement-aa0e9b58-b276-494c-a321-12474dab47fd\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> could also be mistaken as vulval lymphangioma circumscriptum and ectopic <span id=\"urn:enhancement-d8ddfec8-09d9-4ac1-beed-5fb6fd13faf6\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/sebaceous-glands\">sebaceous glands<\/span>. Among children, the differential diagnosis includes warts, syringoma and closed comedones.<\/p>\n<p><strong>Management of MC<\/strong><\/p>\n<p>As the infection can cure on its own, the treatment usually is not initiated especially in children. Among some of the individuals, the need for a therapy is required because of an underlying atopic disease. According to CDC, the treatment options of <span id=\"urn:enhancement-7a055999-26b0-4f44-b922-7cc5dec107ca\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> include physical removal including initiation of cryotherapy and laser therapy. Among children, the use of oral cimetidine is recommended as a safe management of <span id=\"urn:enhancement-a725be84-d51b-4831-ac58-a102c473e8ac\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span>. Podophyllotoxin cream (0.5%), iodine, <span id=\"urn:enhancement-08d85dda-2878-4f64-a276-9fcd9e85da6e\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/potassium-hydroxide\">potassium hydroxide<\/span>, imiquimod and cantharidin are recommended as alternative options.<\/p>\n<p>According to a recent publication titled \u2018Treatments for Molluscum Contagiosum, a Common Viral Infection in Children\u2019, a study assessed the effects of various treatments and management strategies for <span id=\"urn:enhancement-1d39bfca-597e-4d87-a38b-f7f2e6903d43\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span>. It studied 22 trials conducted to evaluate and compare different treatment options for <span id=\"urn:enhancement-8d209be0-b7d2-4d17-ad54-16f7b9f52565\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span>. It indicates that several common treatments for <span id=\"urn:enhancement-06667825-1873-4ac5-aa9d-cadff4b77403\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> such as physical destruction are not sufficiently evaluated while other treatments were not part of the standard practice. The evidence found the infection not favoring any one single treatment and that natural resolution of <span id=\"urn:enhancement-3138ace0-513d-49e5-b8d2-50b9e07f5b48\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> continues to remain as an initial strategy for dealing with the condition.<\/p>\n<p><strong>Alternative Therapy with use of Pulsed-Dye Laser <\/strong><\/p>\n<p>In a systematic review conducted to determine the use of Pulsed-Dye laser (PDL) therapy for the treatment of <span id=\"urn:enhancement-c5bb2bae-659b-4faf-af5d-fe41f0069000\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span>, a search of National Library of Medicine\u2019s database was performed that discussed the treatment of <span id=\"urn:enhancement-593ff102-3c27-4408-9fda-82f0be4d3f48\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> with PDL. In these articles, 161 patients were reviewed and over 4200 <span id=\"urn:enhancement-9bf7ea98-9f8d-49d5-aea6-c225a301ecb1\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> lesions were treated with PDL. The review concluded that PDL offers an effective treatment for <span id=\"urn:enhancement-ec846312-1864-4cda-a545-05fda77a91c5\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> and is considered safe, effective and well tolerated without causing permanent pigment change.<\/p>\n<p><strong>Outcomes<\/strong><\/p>\n<p><span id=\"urn:enhancement-5dcf1550-f771-4155-bee6-06ac6baa70e1\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/mc\">MC<\/span> is a self-healing disease spontaneously clearing during its natural course with a mean duration for a single lesion as 2 months and the mean duration of the infection continuing up to more than a year or two. Although the lesions can resolve without <span id=\"urn:enhancement-b452899a-d083-4a52-ad82-8373fa825501\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/scarring\">scarring<\/span>, scratching or physically removing the lesions should be avoided. \u00a0In some cases, the lesions may cause disfigurement and persisting for 3 to 5 years although reports of recurrences occurring among one-third individuals were witnessed.
