T. <em>pallidum<\/em> belongs to a family of spiral-shaped bacteria varying from 6 to 15 \u00b5m in length and <span id=\"urn:enhancement-36bd96f3-c255-44c6-a99f-4d1f4e79bd0c\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/dark-field-microscopy\">dark field microscopy<\/span> is used to detect it. <em>T.pallidum<\/em> has an unusually small genome, very sensitive to environmental conditions and can survive in the host for several decades. <span id=\"urn:enhancement-2817d4a6-4563-4456-8e6a-092d6324d722\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">Syphilis<\/span> was the first identified <span id=\"urn:enhancement-4b6f5777-a301-4831-a78a-dcf9ae058f8d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/sexually-transmitted-diseases\">sexually transmitted infections<\/span> (STIs) and since its recognition as a new disease in Europe in the 15<sup>th<\/sup> century, its origin was under the subject of great debate. The therapy used for the disease over centuries included prolonged injections of arsenicals, the use of mercury and bismuth salts. For the affected individuals, if the disease did not kill them, the cure most certainly did. The use of <span id=\"urn:enhancement-038a3461-fa76-4e8b-a609-01d5fe7b9467\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penicillin\">penicillin<\/span> after 1940 for all stage of <span id=\"urn:enhancement-d9053495-240c-4063-bcaf-ddba99049d78\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> reduced the incidence of the disease and the rate of mortality. From 1944 to 1954, the cases of <span id=\"urn:enhancement-42d8b11b-ddca-46d3-b5e4-22b2d73e8434\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> decreased by more than 75% and by 1975, the rate of <span id=\"urn:enhancement-f395b61d-8a0a-4cd3-ad3c-0b68e66dae79\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> had declined by 90%. However, the <span id=\"urn:enhancement-1cad4268-8431-440a-8f13-a075ea1118bd\" class=\"textannotation disambiguated wl-organization\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/centers-for-disease-control-and-prevention\">Centers for Disease Control and Prevention<\/span> data from 2014 indicate an increase in the new cases of <span id=\"urn:enhancement-363d9688-6ff5-40e3-af4b-ba19e50fd6cd\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span>.<\/p>\n<p>After the World War II, and in the late 1980s and early 1999s, the most affected population changed from heterosexual epidemic to <span id=\"urn:enhancement-f5e5d176-4128-4f4f-9b90-745cca26560e\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/msm\">MSM<\/span> (men having sex with men). According to the health officials, 50,000 cases of <span id=\"urn:enhancement-fca4faf6-3aa2-4fcb-b9e0-c7318f7b362d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> were reported in the United States in 2013 of which 17,000 cases are of a primary and secondary stage and 75% were detected among men with same-sex partner.<\/p>\n<p><span id=\"urn:enhancement-959e186f-837c-455f-9c87-41bdd148f1f7\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">Syphilis<\/span> also increases the risk of <span id=\"urn:enhancement-2565bb4e-469b-4488-93ad-0034a9df09a9\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hiv\">HIV<\/span> infection with some evidence suggesting the direct involvement of <em>T.pallidum<\/em> in facilitating its progression. The study also indicates that the genital sores serve as an easy passage for the transmission of <span id=\"urn:enhancement-4df7ec6a-8b29-4aa0-8201-682aa2fc9780\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hiv\">HIV<\/span> infection. This makes it vulnerable particularly for the high-risk population such as <span id=\"urn:enhancement-470846b5-7398-4e4a-afe5-e89248524a2c\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/msm\">MSM<\/span> with <span id=\"urn:enhancement-30e30835-438a-40cd-9e1d-06769d916fe3\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> to also become infected with <span id=\"urn:enhancement-ff5682ae-9eb8-4c53-9381-f65be3a66f1f\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hiv\">HIV<\/span> easily. With the increasing rates of newly diagnosed cases, the need for <span id=\"urn:enhancement-0906c49c-2eb2-4d9e-8714-2e5f7a9afc2f\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> screening for at-risk individuals was reaffirmed by the United States Preventative Health Services Task Force. However, the primary care physicians were unable to accurately determine which individuals belong to that category due to the highly personalized nature of discussing sexuality and sexual behavior of patients.<\/p>\n<p><strong>Mode of action<\/strong><\/p>\n<p><span id=\"urn:enhancement-62a96365-0470-4401-ab35-5d2a7cdd828c\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">Syphilis<\/span> is transmitted by direct contact with a syphilitic sore at anogenital, rarely oral and other nongenital sites. The transmission is possible during sexual contact with an average incubation period from 10 days to 3 months and the disease lasting through months or years progressing in different stages. Even with the lack of metabolic capabilities, <em>T.pallidum<\/em> is capable of invading and surviving in different tissues and organs. Progression to the latent stage may present no visible symptom and tertiary <span id=\"urn:enhancement-8c79ef53-3d00-4437-a568-5de4f977118e\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> can appear after 10 \u2013 30 years after affecting multiple organ system.<\/p>\n<p><strong>Syphilis as a study project<\/strong><\/p>\n<p>The research on <span id=\"urn:enhancement-c302dce7-9f09-4294-85ed-bee801355d2a\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> historically has been considered a special disease as a STD with no effective treatment available for years. The <span id=\"urn:enhancement-8b302d25-ee22-4471-a5e5-2392808b8b94\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> study post-second world war II continued without any ethical standards for several decades. Beginning in 1946, in violation of medical ethics and human rights, more than 5,000 unconsenting Guatemala people were intentionally infected with the aim of monitoring the natural course of the disease when left untreated. Another <span id=\"urn:enhancement-6523507d-2890-4a08-b056-2bed5c9c23af\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> study at Tuskegee involved 600 black men that started in 1932, actually continued for 40 years. Even when the use of <span id=\"urn:enhancement-bd6040c6-a126-449b-b3c0-4d25a6617b47\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penicillin\">penicillin<\/span> became the choice of drug for <span id=\"urn:enhancement-35b862b9-9e97-4122-91da-a7ae5fafcb70\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> in 1947, the participants of the research were not administered the treatment until the 1970s. After the <span id=\"urn:enhancement-6556b59d-12aa-40c9-b26f-c0da084e1e4d\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/tuskegee-study\">Tuskegee study<\/span>, the research practice changed and regulations were passed in 1974 that required voluntary informed consent from all individuals participating in studies. In 1995, National Bioethics Advisory Commission was created and in 2009 the President commission for the study of Bioethical Issues was established. And also, any of the studies supported by the Department of Health Education and Welfare (DHEW) was to be reviewed so they meet the ethical standards.<\/p>\n<p><strong>Screening<\/strong><\/p>\n<p>The initial serologic test for <span id=\"urn:enhancement-1c7ea085-1741-425d-9cba-46131a3a7e85\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> was the Wassermann test developed in 1906 and classified as <span id=\"urn:enhancement-56de6a63-2cd6-48d4-a235-4a15f07b60e3\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/nontreponemal\">nontreponemal<\/span> (NTT). Decades later <span id=\"urn:enhancement-7b9df3c3-f6bb-4b06-b4ae-7283c3f178ea\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/sexually-transmitted-diseases\">venereal disease<\/span> research laboratory (<span id=\"urn:enhancement-b7aa06d9-bb90-4b02-bb1d-64f270ff0518\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/vdrl\">VDRL<\/span>) and Rapid <span id=\"urn:enhancement-595a9490-ab15-45f1-b488-86f94d173afe\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/plasma\">Plasma<\/span> Reagin (RPR) developed and the sensitivities of these tests depend on the stage of the disease and usually shows positive only after several weeks of infection. The RPR and <span id=\"urn:enhancement-c6feb364-5133-4e92-a00d-7e5c6a0608d6\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/vdrl\">VDRL<\/span> show sensitivity of 86% and 78% respectively during primary <span id=\"urn:enhancement-786a348d-ba38-49cb-b43a-b492a8af8348\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> and are 100% during secondary <span id=\"urn:enhancement-c7df3621-5713-401b-a914-0bc29fe140da\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span>.<\/p>\n<p><strong>Treponemal tests (TT)<\/strong> detect specific anti-treponemal antibodies and have higher sensitivity than the NTT and are used as a confirmatory test for <span id=\"urn:enhancement-7dc7be10-5768-413b-91a3-a9cce6d14780\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span>.<\/p>\n<p>For the diagnostic purpose, <span id=\"urn:enhancement-bda464c2-85fc-4822-a126-1d529aa635fe\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/nontreponemal\">nontreponemal<\/span> tests can show false-positive results and hence, individuals with reactive <span id=\"urn:enhancement-4a4e32ae-0dbe-406c-b89e-c2cf2a7ff4b0\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/nontreponemal\">nontreponemal<\/span> tests should receive a treponemal test to confirm the diagnosis. The treponemal antibodies can appear sooner than the <span id=\"urn:enhancement-6c05b771-a5f7-420d-9dbc-d0368c3b827e\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/nontreponemal\">nontreponemal<\/span> antibodies and can be reactive for years among individuals either with or without treatment.<\/p>\n<p><strong>T.pallidum immobilization test (TPI)<\/strong> was a specific test for antitreponemal antibodies followed by more sensitive fluorescent treponemal antibody (FTA) test and later the <span id=\"urn:enhancement-faaafaad-f018-4c91-92fb-e883a2febcc6\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/fta-abs\">FTA-ABS<\/span> test which detects the antibodies bound to <em>T.pallidum<\/em>.<\/p>\n<p><strong>The direct detection<\/strong> of <em><span id=\"urn:enhancement-9125404d-72f4-4416-9115-5940820cf1cf\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/treponema-pallidum\">Treponema pallidum<\/span><\/em> uses various methods for the direct detection of the microorganisms or their DNA which includes Darkfield microscopy, direct fluorescent antibody test for <em>T.pallidum<\/em> (DFA-TP) in the blood fluids, direct test for <em>T.pallidum<\/em> in tissue sections, Rabbit infectivity Test (RIT) and <span id=\"urn:enhancement-87ab683a-8fbb-4b95-a377-1ca51b8dd92d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/polymerase\">Polymerase<\/span> chain reaction (<span id=\"urn:enhancement-d2f3090e-08c2-45d3-9dc9-70455d753c65\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/pcr\">PCR<\/span>).<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Penicillin and alternatives<\/strong><\/p>\n<p>Even after decades, the use of <span id=\"urn:enhancement-e0e3a4e8-c278-4a7a-beca-e146e17c0ce5\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penicillin\">penicillin<\/span> continues to be the preferred treatment for all stage of the disease. However, the guidelines published by the <span id=\"urn:enhancement-b650f25b-cddd-42e8-83f0-2b3882407e8a\" class=\"textannotation disambiguated wl-organization\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/centers-for-disease-control-and-prevention\">Centers for Disease Control and Prevention<\/span> indicate the administration of oral <span id=\"urn:enhancement-9189c3a5-2c56-4e6f-aa5e-18678c584a10\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/doxycycline\">doxycycline<\/span> or <span id=\"urn:enhancement-9191fc25-481e-4473-b4fe-e1935bd23a35\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/tetracycline\">tetracycline<\/span> as the alternative form of treatment in the case of <span id=\"urn:enhancement-8eee7379-c205-422e-9870-78b4928c33fe\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/allergic\">allergic<\/span> reaction to <span id=\"urn:enhancement-47f00790-a2ea-4d13-b426-b3fe1feec3d5\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penicillin\">penicillin<\/span>.<\/p>\n<p>In an effort to develop a single-oral dose medication, a clinical trial in 2010 found <span id=\"urn:enhancement-db54ccab-392e-4dab-9859-88e64ff6ed7a\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/azithromycin\">azithromycin<\/span> as an alternative for <span id=\"urn:enhancement-8b294dcb-052d-47ce-b5ed-5fa4bce4f9d2\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penicillin\">penicillin<\/span> to treat early stage <span id=\"urn:enhancement-b19640ae-8984-4f45-a022-6a11ab2e2844\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span>. The clinical trials also showed efficacy in relation to <span id=\"urn:enhancement-ee75a8d9-39c1-4771-b806-dc9fa2e291b8\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penicillin\">penicillin<\/span>.<\/p>\n<p><strong>Macrolide resistance<\/strong><\/p>\n<p>When the treatment of <span id=\"urn:enhancement-9a8b10ad-5580-451c-a923-0d4a4218846a\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> with <span id=\"urn:enhancement-502e2682-4e76-44a6-843b-0a25f948b660\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/azithromycin\">azithromycin<\/span> proved to be promising, another study of <em>T.pallidum<\/em> samples derived from different locations identified a macrolide-resistant strain which was shown to be associated with the single base mutation in the 23S rRNA <span id=\"urn:enhancement-825c2621-8237-42a8-be2b-1b6e37e32079\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/gene\">gene<\/span>. The macrolide-resistance appeared to be predominantly high in San Fransisco and Dublin. Hence, among cities presenting high rates of macrolide-resistance strains, the treatment option for <span id=\"urn:enhancement-552c59f2-ab8b-433e-9824-bbfd90f8af5e\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> should continue to remain as <span id=\"urn:enhancement-093b991c-84e0-4ab1-a057-b55fd3a04375\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penicillin\">penicillin<\/span>. However, it is essential that patients administered <span id=\"urn:enhancement-8f72a1b6-f29d-4c7f-9321-c94a4d9c1d56\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/azithromycin\">azithromycin<\/span> require to be reevaluated with <span id=\"urn:enhancement-d5543de9-60f5-49a7-99ca-4af6fe682268\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/serological\">serological<\/span> testing.<\/p>\n<p><strong>Vaccine on the horizon<\/strong><\/p>\n<p>For the development of an effective vaccine against <span id=\"urn:enhancement-3506498d-dc7e-47cf-83bb-2334ed54dd63\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span>, several key milestones need to be taken into consideration such as the need for the vaccine candidate to be effective in generating a TH1 response and opsonic antibodies.
