Prenatal Syphilis Screening and Congenital Syphilis<\/strong><\/a><\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>&nbsp;<\/p>\n<p id=\"1\">Syphilis, a stigmatized and greatly feared disease by the society because of the pain and the disfigurement that followed, was the first identified <span id=\"urn:enhancement-4fc8d72e-1be8-466a-aa69-80baa1d9206b\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/sexually-transmitted-diseases\">sexually transmitted infections<\/span> (STIs).<\/p>\n<p id=\"2\">Syphilis, the name itself had originated from the work of Girolamo Fracastoro, a poet and a medical personality, who wrote \u2018<span id=\"urn:enhancement-6e48af13-b49d-4c98-96c9-9d956a94a812\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">Syphilis<\/span> Sive Morbus Gallicus\u2019. The origin of <span id=\"urn:enhancement-2d333e8b-913b-4227-a5dd-cb65de81c0ed\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> was under the subject of various debates before the discovery of the bacterium <span id=\"urn:enhancement-d00ec61c-34be-44cd-bc1b-0e5e13462f70\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/treponema-pallidum\">Treponema pallidum<\/span>.<\/p>\n<p id=\"3\">Until the early 20<sup>th<\/sup> century, one of the very popular hypothesis states that the navigators in Columbus fleet, on their return from the New World had brought the disease back on their return in 1493. For the affected individuals, if the disease did not kill them, the cure mostly certainly did. In an effort to treat the disease, throughout history, different therapeutic agents were used that included plants such as guaiac tree (lat. Guaiacum Officinale), the use of mercury and bismuth salts. Interestingly, a German scientist named Paul <span id=\"urn:enhancement-e4b0a4ef-8a4e-4999-ac76-1e28d201639f\" class=\"textannotation disambiguated wl-person\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/ehrlich\">Ehrlich<\/span>, who received the Nobel Prize in Physiology and medicine in 1908 discovered <span id=\"urn:enhancement-1e06af0e-c651-4279-9c38-63f566424b13\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/arsphenamine\">arsphenamine<\/span> (Salvarsan) but later the safer drug used for the treatment of <span id=\"urn:enhancement-843cc99a-fc74-4f01-9453-ca9fbf6077aa\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> was Neosalvarsan.<\/p>\n<p id=\"4\">The seriousness of the disease led to a national <span id=\"urn:enhancement-4302a45f-fe4d-4a2f-adc7-34df99265d32\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> control campaign in 1938 and the therapy usually included prolonged injections of arsenicals, which was later replaced with the use of <span id=\"urn:enhancement-7ac2628c-81ae-4d28-92b5-b7ca16ca9409\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penicillin\">penicillin<\/span> after 1940 for all stages of <span id=\"urn:enhancement-a63d4871-8831-4877-8096-548bda731752\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> thus reducing the incidence of the disease and the rate of mortality. From 1944 to 1954, the cases of <span id=\"urn:enhancement-c2beea46-aba9-4c57-b047-ab072f660a2d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> decreased by more than 75% and by 1975, the rate of <span id=\"urn:enhancement-90c73fb2-c4ba-41a8-acb8-7e3f8fb99020\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> had declined by nearly 90%.<\/p>\n<p id=\"5\"><strong>Syphilis as a study project<\/strong><\/p>\n<p>The research on <span id=\"urn:enhancement-bb6688b8-e6ce-4f68-8d5a-e95f90821b44\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> historically has been considered a special disease as an <span id=\"urn:enhancement-88e516b5-6b9c-4709-bdd2-9645fd7b9566\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/sexually-transmitted-diseases\">STD<\/span> with no effective treatment available for years. The <span id=\"urn:enhancement-6d61a39e-6b77-47a9-9b3c-02bbb8b1dcb0\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> study post second world war II continued without any ethical standards for several decades. One of the notable projects initiated by the US <span id=\"urn:enhancement-af0ee91e-ee1c-45ed-bc6e-9da62d8d49b7\" class=\"textannotation disambiguated wl-organization\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/public-health-service\">Public Health Service<\/span> was the <span id=\"urn:enhancement-21be08e3-56c0-4294-b315-f23d18a12b5c\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> study with the aim of monitoring the natural course of the disease when left untreated. This study involved adults who were deliberately inoculated with the infectious material between 1946 and 1948 in Guatemala and between 1932 and 1972 in Tuskegee. Even with the use of <span id=\"urn:enhancement-2b740a9b-23e9-4e1e-b4cd-f2fcb1354809\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penicillin\">penicillin<\/span> therapy in the 1940s, the participants of the project were not administered treatment until the 1970s.<\/p>\n<p>Since the high rates of infection in the period of World War II, a great progress was made to effectively manage the disease. However, the <span id=\"urn:enhancement-efc5f1ab-40d6-4827-9b51-419538cb20d5\" class=\"textannotation disambiguated wl-organization\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/centers-for-disease-control-and-prevention\">Centers for Disease Control and Prevention<\/span> data from 2014 indicate an increase in the new cases of <span id=\"urn:enhancement-fbc4ba4b-4ce6-4533-8d59-726549d3c335\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span>. After the World War II, and in the late 1980s and early 1990s, the most affected population changed from heterosexual epidemic to <span id=\"urn:enhancement-b4fc1e83-f3c8-42c0-8680-7ee8caaad48d\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/msm\">MSM<\/span> (men having sex with men). Also, the number of <span id=\"urn:enhancement-cd7fc140-5888-44b6-82ee-79dde4c277fd\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/congenital\">congenital<\/span> <span id=\"urn:enhancement-81bb0e21-8c49-48c2-8b7d-e08c50103ce0\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> cases also appeared to be on the rise.<\/p>\n<p id=\"6\"><strong>Incidence<\/strong><\/p>\n<p><span id=\"urn:enhancement-d3ce9251-0d24-479e-a7d5-e66546402c80\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">Syphilis<\/span> is still prevalent in our global society and according to the health officials, 50,000 cases of <span id=\"urn:enhancement-d112c37b-92ac-4a95-aebe-b5cbc7aa562a\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> were reported in the United States in 2013 of which 17,000 cases are of primary and secondary stage and 75% were detected among men with the same sex partner. In 2017, new cases of <span id=\"urn:enhancement-72e98399-100a-47b9-80ca-8df92e2c2b76\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> were estimated at 101,567 compared to the 39,782 cases of <span id=\"urn:enhancement-7983faa0-278d-4929-bbf2-4a53d62411a4\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/hiv\">HIV<\/span> infection in 2016 according to <span id=\"urn:enhancement-849d151d-828a-45f7-a8a2-58581767b437\" class=\"textannotation disambiguated wl-organization\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/centers-for-disease-control-and-prevention\">Centers for Disease Control and Prevention<\/span> (CDC). With the increasing rates of newly diagnosed cases, the need for <span id=\"urn:enhancement-9b2e5f8f-2ead-457d-bcca-06f64aae9842\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> screening for at-risk individuals was reaffirmed by the United States Preventative Health Services Task Force. However, the primary care physicians were unable to accurately determine which individuals belong to that category due to the highly personalized nature of discussing sexuality and sexual behavior of patients.<\/p>\n<p id=\"7\"><strong>Screening<\/strong><\/p>\n<p>With the discovery of <span id=\"urn:enhancement-1b08ee6b-47b7-44f2-a121-5339ea99e33f\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/treponema-pallidum\">Treponema pallidum<\/span>, the effective diagnostic test known as Wassermann test was developed in 1906.\u00a0 However, in order to diagnose the diseases, the complexities continue to challenge the clinicians.<\/p>\n<p><strong>The <span id=\"urn:enhancement-a4b754e6-dcff-41c2-ae9b-c6d9a1ea3eee\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/serological\">serological<\/span> tests<\/strong> with the detection of antibodies against T.pallidum, remain the most reliable form of diagnosis of <span id=\"urn:enhancement-a1da30ca-8e68-41d4-a69e-8f126d5e9d60\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span>, regardless of the stage of infection. Historically, the initial test was a complement fixation test and the <span id=\"urn:enhancement-777c7a5d-9721-4573-a298-cd74a7c06c21\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/antigen\">antigen<\/span> was from the liver of an infant who died of <span id=\"urn:enhancement-6628037c-64b0-4894-b3d3-c04a46dd5b64\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/congenital\">congenital<\/span> <span id=\"urn:enhancement-904b8456-b476-43bb-a3cd-fb6cdd79c592\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span>. <strong>The common nontreponemal (NTT)<\/strong> screening tests include the <span id=\"urn:enhancement-5cd4ad2e-be4a-4e54-b7bc-52cce4a474c7\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/sexually-transmitted-diseases\">venereal disease<\/span> research laboratory (<span id=\"urn:enhancement-99a62f16-e976-4e7a-ade5-82ff0dcf3be4\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/vdrl\">VDRL<\/span>) and the Rapid <span id=\"urn:enhancement-859c11ef-987a-4ad0-bac1-a55793c3b562\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/plasma\">Plasma<\/span> Reagin (RPR) which was later developed, both of which detects the antibodies among the patients with <span id=\"urn:enhancement-e917b12a-d2d8-4964-9fae-ed69389db692\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span>.\u00a0 The NTT can commonly detect antibodies as early as 6 days post-infection and are used globally.<\/p>\n<p><strong>Treponemal tests (TT)<\/strong> detect specific anti-treponemal antibodies and have higher sensitivity than the NTT and are used as a confirmatory test for <span id=\"urn:enhancement-5365ca07-9d18-4876-830f-95deaa1ca516\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span>. The treponemal tests can be reactive for years among individuals either with or without treatment.<\/p>\n<p id=\"8\">The direct detection of <span id=\"urn:enhancement-45bdade0-c787-44fb-b69b-5039c032aa02\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/treponema-pallidum\">Treponema pallidum<\/span> uses various methods for the direct detection of the microorganisms or their DNA which includes Darkfield microscopy, direct fluorescent antibody test for T.pallidum (DFA-TP) in the blood fluids, direct test for T.pallidum in tissue sections, Rabbit infectivity Test (RIT) and <span id=\"urn:enhancement-e2ca5a3b-958f-415f-a670-2a43b18a9a5d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/polymerase\">Polymerase<\/span> chain reaction (<span id=\"urn:enhancement-232e1183-67cf-46d8-9619-c4e1af84a4ac\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/pcr\">PCR<\/span>).<\/p>\n<p id=\"9\"><strong>Prenatal syphilis screening and <span id=\"urn:enhancement-c42f3e3f-43f0-485a-b19c-f5c7dbbdd604\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/congenital\">congenital<\/span> <span id=\"urn:enhancement-51cba90b-0162-47a3-b8e3-389da15d7230\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span><\/strong><\/p>\n<p><span id=\"urn:enhancement-2c31dc8f-8504-4199-aeb3-6a12be54812a\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/congenital\">Congenital<\/span> <span id=\"urn:enhancement-a1e3f454-6c4a-41c5-92ce-f64f58ce6f93\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> presents as two clinical syndromes; early (diagnosed during the first 2 years) and late (after 2 years of life). The prevention of the <span id=\"urn:enhancement-2cebbdd3-4287-476e-81ba-01e301d27f2d\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/congenital\">congenital<\/span> <span id=\"urn:enhancement-241392fa-95a2-444c-a94d-a5d16e7fe53e\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> is based on identifying the disease among pregnant women with the initial screening done in the first trimester and repeated at 28 weeks and also among women in high risk of acquiring the disease such as sex trade workers. This screening can prevent the transmission of the disease from the mother to the <span id=\"urn:enhancement-75d58a15-a8d0-4dbd-9a5c-619afebf7b4b\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/fetus\">fetus<\/span> by treating the mother in pregnancy. However, the detection at near term or at delivery can help with the early treatment of the disease.<\/p>\n<p>As <span id=\"urn:enhancement-69c83cd1-289b-4a48-9a19-4a18651999d6\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">Syphilis<\/span> appears to be on the rise, particularly affecting the <span id=\"urn:enhancement-aa94a520-c0d9-4126-85a7-f11c3479945b\" class=\"textannotation disambiguated wl-creative-work\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/msm\">MSM<\/span> population and infants, screening for <span id=\"urn:enhancement-eed2da75-2862-41b1-a4c9-6b3a888d2e31\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span> in high-risk populations are often recommended. It is also equally essential to understand the sexual behaviors of the patients so counsel, services and appropriate testing can be provided.<\/p>\n<p><strong>\u00a0<\/strong><strong>Penicillin<\/strong><\/p>\n<p>Although <span id=\"urn:enhancement-9d6d3f67-158a-42f8-b0a4-2d7c57a214b5\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penicillin\">Penicillin<\/span> G is the preferred medication used for the treatment of all stages of <span id=\"urn:enhancement-df3569bc-aa2d-41ab-8cc8-25442ec6cec2\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/syphilis\">syphilis<\/span>, the dosage and the duration is based on the stage and the clinical presentation of the disease. The preparation used for the <span id=\"urn:enhancement-441e062b-e14f-4c2a-8ff5-d21a8e867b50\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penicillin\">penicillin<\/span> is also essential to combat the disease as T.pallidum can thrive in sites that cannot properly be accessed by certain forms of <span id=\"urn:enhancement-d41e16c9-398f-4929-badf-dbc31b19ea88\" class=\"textannotation disambiguated wl-thing\" itemid=\"https:\/\/data.wordlift.io\/wl1503301\/entity\/penicillin\">penicillin<\/span>.
